The information and responses in this form will serve as the official club application into the Midwest Conference Boys Club vs. Club competition for the 2022-23 seasonal year. The application period closes on Wednesday May 11. Successful clubs will be notified by Monday May 16.

Club Obligations: Current financial obligations include: team registration fee ($750 per team*); referee fees (see Conference Operating Procedures for referee fees); home hosting fees (athletic trainers, field rentals, video recording, hydration stations, team tents,); all team travel costs.
*state associations may charge an additional state association fee, please check with your state association for this fee.
If accepted, our club will field a team at all five age groups of 15U, 16U, 17U, 18U and 19U for the duration of our time in Midwest Conference Club vs. Club.
Competitive level: Our club will field the most competitive possible teams in the Club vs. Club competition in order to maintain the integrity of the top tier of conference competition. Our club understands that failure to maintain the standards to compete in this league will result in our position being evaluated by the USYS National League management group.
Completion of Club vs. Club season: All Club vs. Club teams in our club are required to participate in the entire Club vs. Club season. Should any team from our club in Club vs. Club fail to complete their league season schedule, the club may lose its place in Club vs. Club.
Club leaders group: Our club will designate a Club vs. Club representative to serve on a leaders group.
Marketing and promotion: Our club will actively promote Club vs. Club through all channels of communication (website, social media, email, newsletters) and share Club vs. Club content provided by or posted by USYS through all of our social media channels.
Policies, rules and procedures: All teams in our club will abide by conference rules and procedures.
The applying club will be properly registered with US Youth Soccer for the 2022-23 seasonal year. All information in this application is correct, and the club understands that any misrepresentation of team accomplishments may result in the club being disqualified from participation.
Club status: I understand that the club finishing lowest in the standings according to aggregate finish across all ages may be subject to relegation into the Team vs. Team format of the conference.
